Here are the forms that are needed in order for us to proceed with an evaluation of your data recovery need. The same forms are used for either the free evaluation or the cleanroom evaluation. For a free evaluation, simply check the appropriate box on the second sheet. These are provided here in the Adobe Acrobat format.
Once you have the forms printed, complete them and send them to us with your drive. If your drive is already in the shop you may fax the forms to 1-800-861-8069.
There are two forms in this package. The first will establish the terms of the contract between us. The second form will enable you to provide a summary of your data loss event.

Test and protect your backups. Don't rest easy just because you have a backup system. It is important to test your backups by restoring them to another machine from time to time. Also, as you practice a backup routine, take copies of your data backups away from the location of the machine. That way, a fire or flood disaster would be less likely to destroy both copies of the data.

Competition Squashed: A drive arrived in our shop for data recovery, complete with a bid sheet from a competitor who acutally quoted a recovery price without even looking at the drive -- proof that the competitor "skims" the market for customers who will agree to pay an outrageous price for the service. The NT drive with NTFS file system was recovered within 72 hours at one-third the price quoted by the competition.
